The western countries in Europe and the United States have embarked on renovating their healthcare services, to allow patient choice through availing price competition between the health care providers that customers can choose from. In the past, healthcare service had been a government dominated area, and in most developing countries it enjoyed a monopolistic position, with ability to provide a variety of services and capture the customer surplus in the market. However, today there are many private organizations that venture in the healthcare service as hospitals and pharmaceuticals, which are highly relying on modern technology to offer the customers quality service. As a result, this has made the cost, quality, and healthcare accessibility a legislative and policy issue that ought to be managed, due to the rising variation. Th e economic benefit of promoting competition and choice is to enhance competition pressure, so that private firms become more efficient, causing them to reduce costs of service and improve their services and products as a way of attracting clients, which is generally healthy and an improvement of the economy (Propper, 2010). New forms of health care financing and delivery have developed as a result of the competitive pressure for cost containment, to slow health care inflation and encourage clients to choose from the available relatively lower cost health care. Hospitals and physicians have been each forming mergers and joint ventures with their partners in the healthcare industry, hence influencing costs and bargaining power as they improve the quality of health care and its distribution to ensure equity in the society (Federal trade commission, 2004). Competition, Patient Choice, and Equity In health care, improving patient and user choices are perceived different in accomplishment of healthcare systems objectives. However, it has become a key concern for most nations so that they get to direct their future in health and public services in regard to patients wants. According to Fotaki, offering patient choice in England is viewed as an attempt to address the failure of the pre-existing policies that had been operational in providing equitable and high quality services to the population, while it is perceived as a way to achieve other desirable goals, for example reducing the waiting lists, hence not an explicit policy objective on its own merit (2007). Steady Oil Price Hike Essay The article about the rising oil price indicates two main economic concepts: first, â€Å"the rule of supply and demand†, and second, that† human wants is insatiable. † Oil is a natural resource and it is created by nature through thousands of years. Time is a very important element in the production of oil. Despite the fact that oil wells and rigs are discovered and/or pumped, still, the natural element of this product is very important. Going back to the relevant economic factors about rising oil prices, there is an unlimited demand for oil while its supply is getting scarcer. Aside from this fact, millions, if not billions, of dollars of investments are needed to have the black gold extracted, refined, and distributed. Everywhere oil is used. It is not only with the vehicles but also with all the machineries producing the goods and even food products that we consume on a daily basis. The second economic factor is the insatiability of humans in terms of their wants. Everyone wants this and everybody wants that. We all consume. We all want to acquire resources. This relates to the article in a sense that: (1) more consumption means more oil consumption; (2) in acquisition of wealth, natural and man-made factors happen in having oil prices rise; and (3) investors in oil trade would always go for a good profit margin while consumers would like to spare resources (money). The first factor is related to the explanation of supply and demand above. The second factor is important because natural calamities such as storm affects oil production but this all goes back to the fact that once the supply of oil is limited, and the demand is big, the market has to balance out and the only way is to raise prices. This benefit the â€Å"wants† of the investors who, in every situation, have their means in having their investments get a very good return. Consumers on the other hand, take their chance in acquiring cheaper oil as they want o save their resources in order to have extra left behind to acquire the rest of their needs and wants and this is explained by having their tanks full before the prices increase. Lastly, oil is not in market where fair competition rules. It is monopolistic. The major factor that makes it so important is its scarceness; its having limited supply with steady demand. First, in the point indicated that â€Å"the oil industry is an example of a perfectly competitive industry† and at the same time noting that â€Å"neither the firm nor its consumers decide on the price, rather the entire industry†. First oil is a product that triggers monopoly whether consumers like it or not. Second, the article contradicts itself: if the industry decides on the pricing itself, not the interaction of the oil firms and their consumers, this means that the market is â€Å"not free†. Not free means NO competition at all. Lastly, the article noted that â€Å"consumers never worried about spending massive amounts of money on oil during an economic struggle† and it is not true. Oil makes everything else expensive and it may be possible that no demonstrations are done against oil but against economic crisis, but it is important to realize that at least 70% of that crisis can be attributed to oil price movement: your cereal will be expensive once oil used in machineries producing it becomes expensive! Argument number two is also very good emphasizing the monopolistic characteristic of oil as a consumer product. The article even dwelled on deeply going into criteria of monopoly to establish the hypothesis, that indeed, this product is never a competitive product, in a sense that whether consumers like it or not, they are just forced to acquire oil out of necessity. The article is very â€Å"narrow† though. Maybe a little bit of expansion as to the idea of having it monopolistic and other related factors would even make the idea of the response more interesting. Lastly, response number 3 is a very good work. Admittedly, my response runs on parallel path as this one. It emphasizes the scarcity of the product, the inelasticity of demand, which is always there, despite the presence of supply or not. It focuses in the effect of transportation though, just like what was written in the article, not like what I have though to widen my perspective in reacting to oil-related write up.

The Lost Generation in The Sun Also Rises The book The Sun Also Rises, by Ernest Hemingway is a perfect example of what life was like after the war. It was about unrealistic love of a young Lady Brett Ashley, and the post war adventures of Jake Barnes and his friends. "In an age of moral bankruptcy, spiritual dissolution, unrealized love, and vanishing illusion, this is the lost generation," and that is exactly what Hemmingway writes about in The Sun Also Rises. Jake Barnes lived a real casual life style. He is a writer that goes out with his buddies almost every night, and drinks. "Oh Jake", Brett said," we could have had such a damned good time together." His only true desire is a Lady Brett Ashley, he is madly in love with her but all he can do is sit back and watch her toy with the other men. Many times he has purposed his love to Brett, she loves him as well, but will not have a romantic relationship with him because her sexual desire is too great and Jake cannot have a sexual relatioship because he has no penis, it was castrated during the war. Jake was also an "aficionado" of bull fighting during one of his fiascos he and his friends and Lady Brett Ashley went to Pamplona. First Brett introduced her new fiance named Mike, a war veteran and a bankrupt drunk. Mike would later get on Robert Cohn's case for bugging Brett and acting like a stalker. Cohn and Brett had a brief fling in San Sebastian, but Cohn could not excpet that Brett had no emotional feelings for him and that love affair meant anything. Cohn doesn't realize that Brett is a permiscius lady. During that week, Jake, Mike, and Bill (Jake's Friend) relize how much they hate Cohn and they all start to antagonize him. While at the bullfights, Brett falls in love with a young "matador" named Pedro Romero, Jake helps her get involved with Romero, Cohn finds out and beats up Jake, and Romero, and then Cohn leave Spain and goes back. Brett realizes that she will only do Romero harm and asks Jake to take her away from him. It seems everytime Brett would get in trouble Jake would always come to the rescue, because he is hopelessly in love with her.

Information about Employment Rights and Responsibilities ERR is a compulsory part of all Apprenticeship programmes to show that the Apprentice has had a full induction to the company or training programme, and are aware of those right and responsibilities that are essential in the workplace. Apprentices may be in the first job or be experienced in their role or industry, but this does not affect their ability to cover the knowledge required. How should it be delivered? The ERR knowledge has been presented in a similar format as the NOS and NVQ units so that it has the same flexibility as the NVQ. There are a number of evidence sources for ERR and these can include: †¢ Referencing evidence showing knowledge of the in-house company induction against the ERR requirements (e. g. Copies of induction handbook, with explanation from the apprentice where necessary) †¢ Collecting a portfolio of evidence, integrated with NVQ and Key Skills portfolios to prove knowledge. †¢ Some technical certificates include an externally tested ERR module, which will cover the ERR. How much evidence should I provide? We advise that the Apprentice should provide evidence showing their understanding of the company procedures and how these relate to the laws and regulations. However, the Apprentice should not be expected to provide large paper documents regarding any parts of legislation; we, therefore, suggest that evidence (e. g. company procedures, websites etc. ) should be sign-posted wherever possible and practical. What is included in this document? The document includes the required standards in the same layout as the National Occupational Standards are produced. This can then be re-formatted in to the same delivery method used by providers for the relevant NVQ. This also includes helpful resources, mainly in the form of website addresses for apprentices and employers to research any information that is not currently included in the company induction. What proof is required by e-skills UK? Once the candidate has completed the ERR, then the candidate, employer and training provider should sign the final page, and send with the Certificate Request form to e-skills UK. Please be aware that while this Unit appears in the same format as an AOC or Unit, it does not need to be assessed or verified in the same way as the NVQ part of the Apprenticeship. Anyone with the appropriate technical knowledge on the subject can be responsible for judging the evidence provided by the candidate. What is the difference between Apprenticeship and Advanced Apprenticeship ERR? While the knowledge requirements are the same for both levels, the Advanced Apprentice will be expected to have a broader knowledge of the ERR, and the employer’s responsibility in this. This should be in line with the greater depth of knowledge required for both the NVQ and Technical Certificates for Advanced Apprentices. Ronald Andersen wrote in the Journal of Health and Social Behavior there is a model of behavior that determines the seeking out of health services (Anderson, 1995). Anderson collaborated with Odin Anderson and wrote The Initial Behavioral Model in 1968 to assist in the analysis of national survey data collected by the Center for Health Administration Studies and the National Opinion Research Center at the University of Chicago (Andersen Anderson, 1967). The individual has to decide whether it is better to quit work and get assistance for medical care or to continue struggling and still not make the grade. There have been no major issues personally with the implementation of the health care expansion. The insurances that I carry are both the Medicare and Medicaid. There has not been any specific positive or negative effect with this coverage. Medicare has provided successfully for preventive measures in my health services such as yearly mammograms, colonoscopy testing every two years (if deemed necessary through findings during prior examination), and comprehensive yearly physical examinations. Unfortunately, there is minimal dental coverage and no eye examinations available. There has been major skepticism with the Affordable Care Act being voted on and implemented in the governmental realm. It is exhibiting a number of positive actions implemented with the creation of this act. It is time that this country gives thought to the individuals which are suffering because of no fault of their own other than being on the lower end of the income scale. Building deterioration is mostly depends on what type of building the time period, building is maintain. The main example we can take is, when we talking about the external walls of the building most of the time this walls get caught to the rain, frost etc. so when we are not doing the maintenance in time period the entire building will deterioration due to the cause. We need to provide special people for the cleaning. If we not provid e special workers for this it will be an issue for the building. Other thing is we need use correct cleaning materials and techniques for a building. Because most site supervisors are not works up for these cleaning things. So worker not use correct materials and equipment. Sometimes it also deteriorates the building. The thing is before we starting the project we have to inform or we have to give advice for the workers about keeping the site clean when you are finishing the work. Then we can avoid from this cause. The other thing we can see is ignorance of the causes of deterioration and decay. It means some workers they dont know the effect of the things such as moisture, fire etc. they dont know these things are cause of deterioration in building. Because most of the bass other helpers are not well educated people. They know only the basic stuffs of their professions. So they work without any risks. But some are very ignorance people, means they dont care about those rules stuffs. They do what they like to do. So the thing is site manager or the site supervisor need to handle these problems. Most of the people dont have any plan after the project. Why we saying like this is for an example when we take a lift. We need to have maintained every 14 months after it fixed. Because when the lift is working the lift cables are ablate. So when we not maintain the lifts again the hazards are over coming. Likewise we need to have proper maintains to the buildings such as we need to paint the house every 6 month time or 1 year time, we need to check the plumbing works we need to checks the electrical works etc. The other thing we see is adopting a negative attitude of waiting until emergency measures are required. It means most people are waiting until accident happen to start their maintenance. Because they are not thinking about the future, if this happens what happens to the whole building. Even in the site also this issue happens like imagine the scaffoldings are not check before starting the work. Everyone knows there was a hazard we can expect. But the supervisor doesnt take any reaction for this issue. Basically what happens is when workers are working on the scaffold, if scaffoldings are not fixed all the workers on the scaffolding in danger. So we need to check before it will cause an accident. We can specify these issues under faulty of construction. Because most of the causes of deteriorations are based on human. When we talking about construction during any construction work we have to do certain things such as excavating trenches, foundation, concrete putting, form work, masonry work etc. we all know these things are doing according to a standard specification. Because when we are not constructing those things including high standard the entire building can deteriorate in 2-3 years. For an example we need to excavate 5m to see the good soil for the construction. But sometimes the workers are excavating 4m and saying these is that selected soil same as for the trenches. So we need to have lack of super vision for these construction periods. Also we can combine the faulty materials under faulty construction and also the faulty systems. Talking about faulty systems it is mainly based on building services engineering. Because sometimes the building engineers are not looking at the drawing they doing with their experience. But most of the time if one single wire is missing the entire electrical work will be jammed and it will be serious accident. Not only for electrical these issues can happens in many services such as gas supply, water supply, has communications etc. so these thing are deteriorated the building. When we talking about faulty materials failure of client, architect, builder, designer reject substandard materials. The other most common issue is in adequate inspection of material by supplier or receiver. For these issues first wrong point is planning. As before we discuss before we starting a project or ongoing a project we need have a plan. Most of the site what the do is when the materials are finished then only them buying the other load of materials. But some sites what they do is they bring up the materials for every one weak. So the wastage or the other issues are not over coming. Other big issue is in site there is no selected place to unload the materials. So most of the time transport cost is very mush of higher. So faulty materials are mainly cause for the building deterioration. When we talking about the moisture atmospheric, they also can harmful for the building can deteriorate the building. Talking about the moisture, penetration of the external fabric or through the ground floor constructions giving to dampness which may create a suitable condition for fungi growth and attack. So it will harmful for the foundations the building materials. So what we can do is keep the building materials in dry states reduce the deterioration in building. Sometimes faulty plumbing also can harmful for building. Because sometimes when the plumbing works do out of copper or steel, chemicals of the soil react with the pipes. Because of that that bearing capacity of the soil reduces it can deteriorate the structure of the building and foundation. Also we know when underground pipe leaks happen that also cause for the structure of the building. When we talking about atmospheric issues wind, rain sun are mainly occur to the reaction of the structural form of the building. It can mainly cause for the structure and mainly for external finishes. Because when rain falls it hit to the external walls and the moisture, water will effect to the wall. The other thing is in foreign counties, the atmospheric condition is cold and snowy weather. So this kind of whether type is mainly cause for the building and for the roofing structure. Talking about faulty design poor detailing at the design stage, insufficient allowance for expansion or contraction, incorrect place damp proof courses, poor jointing between different materials and components, poor specifications, lack of adequate considerations of future maintenance are main issues for deteriorating building. The main thing is after putting the foundation we need to have place damp proof cause. Why we need damp proof cause is when we not putting DPC the water will go through the foundation and rubble work things. So it will mainly cause for the structure of the building. Other thing is when we doing a design we need to have proper detail plan including specifications such as plumbing, electrical works etc. so when there is no proper details on the plan sometimes the workers cannot do their work properly. Sometimes plumbing workers cant do their plumbing things correctly because sometimes when we are doing the plumbing recently we are founding obstracles.so they ca nt do their work properly. So faulty design also deteriorate in building. We can see all these issues mainly cause for the building deteriorations. What we can do is minimize these issues and do our work maintain the building every given period of time. It can keep the building maximum time period. It means maintenance is defined as continues protective care of fabric and contents of a building. it may be weekly, monthly or yearly according to the building and nature. When we talking about practice of a conservation most of the people think once the building is completed its does not maintain or look for many years. Why they think like this is they spend large amount of money for this project and the through that the contractor do their work well and no need to maintain. But many major repairs to buildings could not have been prevented if buildings are properly maintained. Building maintenance has become major part in construction industry earlier days now a day. Because in earlier taking about historical building they are not such historical buildi ngs, they are building up and as well as they programme the future maintenance also. Because of that we can see the historical building same as past years. Taking about more of building maintenance we can categorized such as cyclical, planned and reactive maintenance. Cyclical maintenance is starting with daily routine and working up with a programme of daily, weekly, monthly, yearly and every 5 years. It means that cyclical maintenance is carried out to preserve the facades of the building and ensure that the fittings and fixtures remaining in good condition. For every five year routine architect and surveyor is responsible to check the defect of the building. The types of cyclical maintenance carried out under local authority. For the surveyors and architects have to look for things and checking such as replacement of water pipes and tanks, replacement of booster and transfer pumps, cleaning out all voids places, checking the electrical installations, checking out whether there is a deterioration sign in the building, cleaning out gutters etc. additionally if the client wants we can do things such as repairing and redecorating, rewir ing, reroofing etc. depending on the nature of the maintenance cyclical maintenance can divide in to two categories such as day to day maintenance and maintenance including builders work. When we plan the maintenance it will extend the life and preserve the appearance of the building. It is the deepest lake in the United States and is the seventh deepest in the world. Crater Lake has an average diameter of 5.3 miles in length and is approximately 1,932 feet deep.   Ã‚  Ã‚  Ã‚  Ã‚  Crater Lake is a result of a volcanic explosion that happened about 7,000 years ago. A long time ago, the pacific oceanic plate was gradually moving under the pacific continental plate in the process of plate tectonics. The pressure shaped the land to move upward and create a line of mountains that are currently located on the Cascade Range. These lavas piled and cooled on top of each other resulting in mountains like Mazama and Hood. Mount Mazama was built by successive flows of both andesite and dacite lavas. Mount Mazama was a stratovolcano, which was about 12,000 feet high after series of ash, cinders, and pumice explosions built it upward. During it?s final eruption the magma chamber was emptying and the underlying support for the mountain was lost and the walls of the volcano began to collapse. In just a matter of days the top of the mountain was destroyed and left a caldera. A caldera is a word used by geologists to describe large basin-shaped volcanic depr essions. All staff should understand their own responsibility to bring matters of concern about children, fellow teachers or volunteers to the attention of the Head Teacher, Governors or Hampshire County Council. In these situations I feel it is my duty to try to reach out to each patient from where they are coming room. If I have a patient who does not speak English utilize a trained interpreter or our Curaà §ao phone to help communicate with my patient. This can put our providers behind schedule.It is very tempting in these situations to allow an employee who is not a trained interpreter but speaks the same language, or a family member to interpret. However, this does not follow clinic protocol and therefore is unethical. I work n an internal medicine clinic and most of our patients have multiple health issues. It would be very unfortunate if we missed a subtle but important change in the patient's health history because we didn't wait for the trained medical interpreter.The ethical dilemma in this situation is whether I should wait for the appropriate interpreter and potentially run the risk of frustrating sick patients who have to wait a long time to see their backed up provider, frustrate staff members who may not get a full lunch break or have to stay late because or utilize an inappropriate interpreter and take the chance of going something important in the translation. In my opinion, everyone deserves the best possible medical care regardless of where they are from or what language they speak. Legalize that morals are a very personal thing and that my morals may be different from the people I am caring for. I cannot let this change the way treat my patients. Sometimes this can be very difficult. When I worked in the operating room We had many traumas involving gang members who had been shot while committing a crime. We still had to work just as hard to save these people as we would anyone else. We used a lot of resources and did what we could to save them. I always found it startling how the staff members in the OR would talk about these people as we worked on them.I get that they are criminals but they are also human beings. Are job was to keep them alive, not judge them. This said, I had a situation that really caught me off guard and made it very difficult for my to be kind to this patient. I grew up in a predominantly Jewish neighborhood in a suburb of Chicago in the ass's. Almost everyone I knew had a family member who was a concentration camp sun,'ivory. It was an everyday occurrence to see an older person with the dreaded â€Å"tattoo† on their forearms. While interviewing my patient before going back to the OR we were talking and everything was fine.It wasn't until had to check her leg to make sure the surgery site was marked that I noticed she had a portrait tattoo of Doll Hitler. I was astounded. She had seemed perfectly polite and appropriate. Suddenly I could hardly look at her. I'm sure my voice became very short and clipped. Once we got her to sleep I couldn't stop talking (to anybody who would listen) about how appalled I was with her tattoo and her probable belief system. It wasn't until much later that I realized that I really let my own feelings interfere with the best possible treatment of my patient.The ethical dilemma in this situation is, should we treat unkind people the way we treat we would want to be treated. I feel very strongly about respecting others cultural beliefs, even when it comes to health choices made that may not think are the best thing for the patient. The only time I really struggle with this is when the men in a family want to make health decisions for the women. My personal opinion is â€Å"if it's not your body, stay out of it†. However, unless the woman is being physically abused, it's none of my business.All I can do is give them the best possible information regarding their health, educate them in a culturally sensitive manner and offer them appropriate resources; the rest is up to them. Some days it can be very had to take others morals, values and ethics into consideration. I really try' to remember to treat others the way they want to be treated and not necessarily the way want to be treated. For someone as opinionated as me, this can be a struggle. Everyday is a new chance to do the best I can for my patients and I feel blessed to have the opportunity to do so.
